Map addresses

A01.01 means Stream A, Block 01, Module 01.

Maturity

Tier	Description
T1 Ad-Hoc	Informal or absent.
T2 Controlled	Basic practice exists but runs inconsistently.
T3 Governed	Documented, controlled and consistent.
T4 Measured	Data-driven, with response to deviation.
T5 Adaptive	Continuous improvement, with AI-augmented practice where suitable.

Evidence

Outcome	Meaning

Augment	Agent senses and proves, human decides.
Keep human	Human runs the decision.
Fix first	Process, data or decision rights need uplift.

Permission	Meaning
Observe	Read only.
Recommend	Propose an action for human decision.
Execute within bounds	Act within a bounded permission.

Rule: An agent may hold responsible duties; a human always holds accountable authority.
